Protecting Your Medical Information

front-desk-side-viewOur providers follow guidelines established in the Health Insurance Portability and Accountability Act of 1996 (HIPAA) when using and sharing your protected health information with appropriate care providers and insurance companies. Your health information is only shared when it’s required to provide you with care and cannot be released without your consent.

You can trust us to provide instructions for transferring medical records to and from this office. We are happy to assist you.

Medical Advice

Our clinical associates are available to help you with any medical needs including advice, test results, or other concerns you might have. If you have a critical question to ask your provider, send them an email anytime through Lakeland myChart. Please be advised that our medical providers may ask you to schedule an appointment to better meet your medical needs.

Easy, Online Access to Your Medical Record!

Our practice participates in Lakeland myChart; a free, internet-based tool that provides you with secure access to your medical information that’s released at the discretion of your physicians and medical providers.

Request access by calling 1-800-LAKELAND, or ask a staff member to register you at your next visit.

Patients who are enrolled with Lakeland myChart can check account balances, e-mail their physician for medical advice, manage appointments, pay bills, print copies of their medical record, and refill prescriptions.

Medication Refills

You can request medication refills by calling the office during regular business hours or through Lakeland myChart. Please allow 24 to 48 hours (excluding weekends) for your prescription to be processed by your provider. Let us know when you see other healthcare providers and what medicines they prescribe to you or change. Please be advised that your provider may require you to attend an appointment with them to discuss the progress of your health with your current medication. If your insurance company requires prior authorization, please allow an additional two to three days to process your prescription.

If you need to make an appointment or need to ask our clinicians a question, you can call the Clinic Service Center at 269.463.3600 and they will be more than happy to assist you. 

Referrals

To ease the burden of care coordination for you, we will refer you to specialists, behavioral health resources, home health agencies, and nursing homes. We even monitor those referrals to guarantee that you get the quality care you deserve.

If your provider wants you to see a specialist, our Referral Specialist will make the appointment and contact you with the date, time, and specialist information. If you have questions or have not heard anything within a week then please call, 269.463.3600 and ask to speak with the Referral Specialist.
